Prospective Patients

New Patient Consultations:
​

Those interested in becoming a patient at Integrative Medicine of Central New York, PC should read through this page and then register to become a patient; click here to begin your registration. After completing and submitting the registration, individuals will be contacted by the office to let them know they have been confirmed as a new patient and to discuss the type of new patient visit that is needed. Once registered, patients can access the patient portal directly; click here to access the patient portal. We offer 2 hour new patient consultations and 60-minute IV nutraceutical consultations.

Upon scheduling your initial visit at Integrative Medicine of Central New York, PC, we require a 50% deposit of the fee for your scheduled visit.  Patients are also required to complete a variety of forms and agreements prior to their visit, all of which are available through the patient portal. Patients can access the forms any time they log into the patient portal.

Transferring Care:
​

Dr. Heidi Puc serves as an integrative physician and works in conjunction with your primary care physician, standard oncologist, etc. In most cases, patients are not transferring their care, but adding a new type of care.

How to schedule a visit:
​
Prospective patients should register as a new patient in the patient portal; click here to begin your registration or learn more by scrolling down to "New Patient Information".  After the registration form is confirmed, office visits can be scheduled via the phone or through message in the patient portal.

What to expect on your first visit:
​

Initial visits typically last 60 minutes to 2 hours depending on the type of visit. During this visit, the doctor will go through the details of your health concerns to understand the root cause of your symptoms. She will use this information to determine if any lab tests are needed, and to develop a plan that may include dietary recommendations, supplements, and possibly nutraceutical infusions.

 
What to bring:

Please bring a driver’s license and/or another form of picture ID and your Medical Insurance card(s), as well as any additional medical records that did not get sent prior to the visit.

Patient Portal Information/Directions:

To become a patient of Dr. Heidi Pucs or Jennifer Kohler MS, FNP-C, CLC, at Integrative Medicine of Central New York, PC and have access to the patient portal, prospective patients must first register to be a patient. To do so, hover over the “Patient Information” button at the top of our website and then click on "Patient Registration" within the drop-down menu if viewing on a computer. If viewing on a mobile device, click on the menu button at the top of the screen and then click on the plus sign next to "Patient Information" to access the drop-down menu. "Patient Registration" will bring you to our patient portal registration page or you can click here. A new browser tab will open up; follow the directions there to complete registration.

Once registered, patients can login to the patient portal directly from a computer by hovering over the "Patient Information" button at the top of our website and then clicking on "Patient Portal" or by clicking here.  Patients can login to the patient portal directly from a mobile device by clicking on the menu button at the top of the page. Patients can access their information and set up appointments in the patient portal.
Sending Medical Records:
​

Patients may upload medical records directly through the patient portal for physician review or have their other providers fax records to us at (315) 741-5770. Due to the sensitive nature of medical records, we ask that medical records are not emailed. Patients are also welcome to mail their records or drop off in person to have them scanned into their patient portal.

Email Use & HIPAA Compliance:
​

In order to ensure that patient confidentiality and medical records remain secure and HIPAA compliant, we ask that patients do not email medical records or communicate via email about medical conditions/situations. All communication of a sensitive nature should occur through our patient portal to maintain HIPAA compliance.

​

Forms:
​

All patient forms are found directly in our patient portal where they are completed and saved electronically.  Patients can access these any time they log into the patient portal.

​

Payment Options:
​

Integrative Medicine of Central New York, PC accepts cash, check, and major credit cards. We do not accept insurance, but a Flexible Spending Account (FSA) or Health Savings Account (HSA) may be used, depending on your account. Please call us to ask about financing options.

Insurance Information & Self-Submission:
​

Integrative Medicine of Central New York, PC is a direct pay practice, and does not accept health insurance, including Medicare.

Established Patients

Cancellations & Rescheduling:
​

Patients unable to keep their scheduled appointments must notify the office at least 72 business hours in advance of their appointment so that we may accommodate others who are waiting to be seen. Please note we are not open on Fridays so that is not counted as a business day. Cancellations made within 72 business hours of the appointment will incur a missed appointment fee. Patients needing to reschedule an appointment need to contact the office as soon as possible via phone call or message through the patient portal.

PLEASE NOTE: Our office charges a 30% fee for any visits cancelled within 72 business hours of the visit and/or is a no show for the appointment, this includes all types of visits including infusions.

    

Urgent Care:
​

Patients who are experiencing a medical emergency must call 911, go to urgent care, or their local emergency room to receive care. Dr. Heidi Puc and her associates at Integrative Medicine of Central New York, PC do not provide emergency physical or mental health services and we do not have a doctor on call during our off-hours.

​

Prescription Refill Requests:
​

Patients needing prescription refills must contact the office via phone or patient portal a minimum of 2 business days prior to when the prescription is needed in order to allow time to fill the request.

​

Buying Products Online:
​

Dr. Heidi Puc has an account set up with Full Script (click here to go to Full Script) that allows both patients and visitors of our website to order supplements directly online for home delivery.

FAQS

Do you accept health insurance / Medicare / Medicaid? 

​

Our practice is a direct pay practice. We can provide a “superbill” that can be submitted to your insurance company for possible reimbursement, but we cannot guarantee this.  Our Practitioners have opted out of participating with Medicare so you cannot submit “superbills” to Medicare for reimbursement. Normal standard labs tests, if ordered by our practice would be processed through your insurance plan and may be partially or fully reimbursed, however certain specialty testing must be pre-paid out of pocket, but you could submit it for possible reimbursement from your insurance if you desire.

How much do your services and specializations cost?

​

We have recently added several new types of New Patient visits that start at $721 for the initial visit. Please reach out to us and let us know what your needs are, and we can find the appointment that best fits your situation. You can also simply register as a patient by clicking on the 'BECOME A PATIENT' button in our main menu and follow the directions there. Once you are in the patient portal, you will see the cost of our services and specializations.

If cost is an issue that prevents you from becoming a patient in our practice, you can now apply for a 15-month interest- free credit card to help pay for the services at our practice.  Please look for the Advance Care link on our website. We also now partner with AmeriCu Credit Union with special financing options. Please reach out to us if you would like more information.

Do I have to be a patient to take advantage of services your practice offers?

​

No, we have numerous services that are available to non-patients.

Non-patients can now come to our practice for general wellness infusions without becoming a patient. There is a medical form to complete that needs to be reviewed by a practitioner for a one-time $25 fee.  If you need more complex infusions for treatment of a disease process, you will only need an infusion consult, which ranges from $620 - $712.

​

Non-patients can also take advantage of our Neurofeedback brain mapping and re-training program, Heart Math, and health coaching services.

​

Integrative Medicine of CNY also has high quality supplements available for purchase in our office or to be ordered and shipped to you directly.

Does your practice see children?

​

We have recently expanded the practice to serve patients 9 years of age and up. Our Nurse Practitioner will be seeing our younger population.

Does your practice do Wellness visits?

​

We have recently added Prevention and Wellness visits, and Well Women visits including gyn care.  Our nurse practitioner can provide you with your annual Well Women care and address hormonal health concerns if needed. For those who have been diagnosed with chronic medical issues such as high cholesterol or diabetes or are at risk of developing diabetes,  you can see our nurse practitioner to help improve your cholesterol levels or diabetes management, or you can see one of our health coaches to learn ways to eat better to help control your cholesterol or diabetes. Our team of health coaches can also help with weight loss and customizing wellness plans, including meal planning, to best meet your needs.

Does your practice offer Bio-Identical Hormone Replacement?

​

Yes, we partner with a compounding pharmacy to offer BHRT and can provide consultations, testing and treatment for hormonal health. We can also evaluate hormonal health and discuss if BHRT is right for you or if you prefer treatment with herbal supplements to address hormonal problems.

I was bitten by a tick, and I want to make sure I am treated appropriately. Does your office see patients for acute tick bites?

​

Yes, we do offer Acute Tick Bite appointments for people who have been bitten by a tick within the previous two weeks, which involves evaluation, treatment and comprehensive testing and discussion about Lyme Disease and co-infections.  Please remember to keep the tick and send it to the Upstate Tick Testing lab for free tick testing.

I saw one of your providers in their previous practice; would I be considered a new patient?

​

Yes, you would be considered a new patient for this practice at IM of CNY. We require patients to be seen at least once a year to remain active. If you were previously seen at IM of CNY you would be considered a "re-establishing" patient with a visit duration in-between a new consult and follow-up.

I think your providers are fantastic. Can Integrative Medicine of CNY be my Primary Care Physician?

​

We encourage our patients to have a Primary Care Physician, especially for acute conditions or treatments that our practice cannot see or treat.  We will serve as your specialist in natural and integrative remedies to work in partnership with you and with your other physicians to address your health and well-being from a holistic approach.

I heard about Mistletoe treatments. What exactly is this?

​

Mistletoe (viscum album extract) has been used as medicine for centuries. In latter days it has been available as concentrated extracts and administered mostly by shallow skin injection and even by intravenous method, and this is a therapy that is available at IM of CNY when indicated.

​

Most of the medical uses come under the headings of improving immune performance and the effectiveness of chemotherapy and/or radiation as well as reducing side effects of chemotherapy for cancer and improving quality of life. Most research on the application of mistletoe has been performed in Europe and hence its use is much more common there than in the United States. It is considered “the most well studied integrative cancer therapy in the world with 75-85% of all European cancer patients utilizing this medicine.” 

I heard about Helleborus. What is this?

​

Helleborus niger is a winter-blooming perennial evergreen plant found in European alpine forests. Its beautiful flowers have white petals and yellow-orange centers. It is a medicinal plant, and its extracts are made into homeopathic remedies by the company Helixor according to an anthroposophic therapeutic approach in Germany since 1994. 

​

Helleborus niger is used in the treatment of cancer and other chronic inflammatory diseases such as autoimmune disease and chronic Lyme disease around the world. Most commonly it is used as a complement to mistletoe therapy but can also be used by itself in circumstances where mistletoe is not tolerated (ie, in highly inflammatory conditions). Interest is growing in the application of Helleborus niger for the treatment in disorders of the central nervous system (CNS), neurological trauma, Parkinson’s disease, dementia, post radiation and chemotherapy fatigue, endometriosis, edema, acute arthrosis (inflamed joints) and rheumatoid arthritis as well as Kaposi’s sarcoma and respiratory infections in AIDS patients. Several case studies on anxiety towards the end of life has encouraged use of this medicine in palliative care settings as well. There is also interest in the application of Helleborus for Lyme disease and CNS symptoms related to autistic spectrum disorders.
